Permit applications and forms

​Johnson County completes all inspections for Shueyville properties obtaining a building permit.  Applicants are required to submit BOTH Shueyville and Johnson County building permit applications. 
The city permit can be downloaded below and submitted via email to the Shueyville City Clerk at [email protected].  The Johnson County permit can be completed online through the CitizenServe Portal.

• Permit Application for alterations, additions, and renovations to existing structures.
• Building Permit for all zoning districts, except use as one and two family dwellings
• ​Residential Building Permit Application for new one and two family dwelling construction.
  • ​​​​Lake Woods HOA- the City must have sign off from HOA before issuing a permit  ​
